University Park Radioactive Waste Form

To request a radioactive waste pickup, complete this form and click the "submit request" button below.

To request empty containers only, you just need to complete the Empty Containers section below and click on the "Submit Request" button below.

Please allow three days for your waste to be collected. The waste will not be collected if the waste cards are not filled out completely. If no one shows up in three days, please call us at 865-6391.
