Due to circumstances beyond our control, the on-line radioactive waste pickup request form that you have been using for the past few years is no longer working. The requests appear to be submitted, but the final forms have not been received at our office. Until the problem is corrected, please download this blank radioactive waste pickup request document, fill in the appropriate information, and fax it to us at 865-7225. You do not have to complete the "MBq" or "Other Hazards" fields in the tables.
